What types of services does Brooke Care, Inc. provide?

Brooke Care provides a variety of services. Each service is outlined in detail in this website (see link to your left entitled "Our Services"). Services that Brooke Care has active contracts through the Springfield Regional Center are as follows: 

  • Residential Group Home
  • Residential Individualized Supported Living (ISL)
  • Day Habilitation (Community Integration)
  • Respite
  • Personal Care Assistant

Our service expands as a need arises. If you do not see a service listed that your family could benefit from please contact Steve Harter and he will look into adding it to the Brooke Care, Inc. contract. 1-800-683-5955.

How do I receive services from Brooke Care Inc.?

There are two methods you can use to receive services from Brooke Care, Inc. The first method involves becoming a client of the Springfield Regional Center in Springfield, Missouri.  To do this you must call the Regional Center toll free at 1-888-549-6635 and schedule an intake. The intake worker will then make a visit to your home and assist you to fill out the required eligibility documentation. A psychiatrist will also attend this meeting and do some standard testing in order to  determine if you/your family member are eligible for services. Once determined eligible you will be assigned a Service Coordinator who will assist you to set up a Personal Plan which outlines the services you are needing. Next you simply request Brooke Care to be your service provider and a meeting will be set up by the Service Coordinator for you to meet with Brooke Care.  After this meeting support staff can be hired and trained and services can then be provided. It's that simple.

The second method involves securing services thru private pay. Anyone is eligible to receive services via this route; simply contact the Brooke Care offices at 1-800-683-5955 and speak to Steve Harter.  Steve will assist you to explore the types of services you need and then help develop your contract and any payment plans.  Once this is complete we can begin the hiring process and you will soon be receiving the services you need.

Can I hire my own support staff?

Yes you can. We believe that the individual receiving services should be empowered to hire their own staff. It makes sense that the individual spending the most time with the support staff be integral in deciding who that support staff will be. We only have to follow a few simple contractual rules regarding staff qualifications.

How can I feel safe and secure about the support staff?

All staff that are hired are subject to thorough criminal background checks as well as a Missouri Motor Vehicle Drivers Detail report. 

In addition we offer extensive training to all of our staff. This ensures they have the skills necessary to provide the highest quality of service to our clients.

Who pays for the services I receive from Brooke Care?

Services that are set up for private pay are paid for by the party(s) that contracted for the services.

If you receive services through the Springfield Regional Center you will most likely have your services paid for by Medicaid/Medicare or private insurance. There is a slight possibility that you may have a small amount of out of pocket expenses, however this is rare and will be determined during the eligibility process.

The Springfield Regional Center is the state agency located in Springfield, Missouri that assists individuals with Developmental Disabilities that reside within their service area to receive supports from service providers such as Brooke Care, Inc. Where can I find the Brooke Care, Inc Main offices?

The Brooke Care offices are located on the west side of the Ava square in Ava, Missouri. 

From Springfield: Take Hwy 60 East toward Cabool. Take the Hwy 5 South exit in Mansfield, MO. Go south/right 12 miles to Ava.  At the stoplight in Ava take a left. Follow this past McDonalds and the Town and Country grocery store, go right around the sharp corner at Pizza Hut and then past the high school and on up the hill to the Ava square. Go on the square and we are located on your right. We're easy to find so please feel free to drop in!
